How to preserve the jawbone after a tooth extraction?

During a tooth extraction procedure, the blood vessels around the tooth rupture and the alveolus (the tooth socket) fills with saliva and blood, from which a blood clot forms.

The bone wound caused by a tooth extraction procedure takes two months to heal. Proper ossification is ensured by a well-preserved height of the alveolar bone and an appropriate density of the newly formed bone.

Clinical studies show that:

  • after a tooth extraction procedure, a severe alveolar bone volume reduction occurs due to the bone and soft tissue resorption,
  •  two-thirds of the affected tissues undergo some degree of resorption during the first three months.

It is primarily the bone facing the cheek which resorbs very quickly, which may cause several problems.

The jawbone preservation is important for:

  • aesthetic reasons,
  • the optimal possibility for subsequent dental implants placement,
  • more effective wearing of dentures,
  • the regeneration after bone loss which happened as a consequence of the periodontal tissues inflammation.

If we replace the missing tooth with a bridge restoration procedure, a gap can form between the bridge and the underlying gums, since the gums are attached to the jawbone and recede as the bone resorbs. 

The formed gap does not look attractive and makes dental hygiene difficult as food particles can accumulate in the gap causing inflammation of the gums.

Placing an implant may stop the bone loss, but an extensive bone augmentation procedure must be carried out first to ensure that the implant is completely surrounded by bone.

Failing to build up the bone can lead to poor aesthetic results. It is, therefore, worth the effort to ensure that the contour of the jawbone is retained as much as possible when teeth are extracted. Bone augmentation can be achieved by using special bone substitute materials, such as Geistlich Bio-Gide, to fill the alveolus.

If necessary, the protective membrane is also used, such as Geistlich Bio-Gide, to protect the wall of the alveolus, which is often damaged during the tooth extraction procedure. The protective membrane protects the damaged area, promotes undisturbed bone regeneration and supports wound healing.

The natural structure of the Geistlich Bio-Oss serves as a scaffold for new bone formation. After four to six months, Geistlich Bio-Oss is intermingled with new bone and is then included in the natural remodelling process of the bone.

The natural contour of the jawbone can therefore be retained and with that a beautiful aesthetic appearance, as, consequently, the gums do not recede either.

It was the development of X-ray diagnostics and special computer programmes that rendered possible the development of guided implant surgery.

CBCT dental radiographic examination or 3D X-ray imaging provides an extremely accurate assessment of a patient’s anatomy, while optical impressions enable a collection of very detailed information about dental and mucous membrane surfaces.

By combining CBCT scans with optical imaging, the medical team is able to make an accurate, reliable, safe and predictable plan of the entire procedure.

After obtaining the CBCT scan, modern computer tools provide an accurate planning of the surgical dental implant placement into a patient’s virtual jawbone.

Apart from determining an accurate implant position, these computer tools also mark the location and the course of nerves and sinusoidal cavities. In addition, the computer tools facilitate quantitative and qualitative ossein analysis and an evaluation of implant positions.

A virtual implant-prosthetic plan is converted into a surgical guide which shall guarantee secure and successful surgical work resulting in accurate dental implant insertion. A surgical guide shall clearly specify a dental implant position, direction and inclination, as well as an implant depth when inserted into a jawbone.

Surgical guides guarantee extremely precise implant placement, with accuracy levels being less than 1 mm, thus ensuring the high reliability of dental implant surgery procedures.

By means of this method, implants can be placed close to vital structures, which enables maximum utilisation of the available jawbone ossein volume.

This provides more opportunity for inserting dental implants at no additional costs that are commonly associated with lengthy bone regeneration processes that ensure sufficient bone volume.

Make sure you have an attractive and radiant smile by caring for your tooth colour with the assistance of professional dental support.

Young people have lighter, shinier teeth and a truly bright smile. Over the years, teeth become darker despite good personal and professional oral hygiene.

Over time, teeth become discoloured or darker for the following reasons:

  • accumulation of natural and artificial colourings from food and beverages
  • smoking
  • poor personal oral hygiene
  • avoidance of professional oral hygiene
  • aging of teeth

External tooth staining occurs when pigmented residue from food and drink get trapped in plaque on tooth surfaces. External tooth stains can be removed with professional dental cleaning.

It is important to know that without the use of active oxygen, teeth can never be as bright and shiny as they used to be. This is due to internal staining resulting from the presence of cracks in the enamel, through which dyes penetrate the enamel where they accumulate over the years.

Internal stains can be removed from tooth enamel by means of active oxygen:

  • by bleaching in the dental office using 6% and 32% hydrogen peroxide
  • by bleaching at home using 10% and 16% carbamide peroxide

In-office teeth whitening

In-office teeth whitening takes 1- 2 hours. The number of visits depends on the individual case. Often, you may only need one visit to achieve an extremely good result, with a smile that is at least four shades brighter. In order to get the best possible results, three visits are required over a period of two months.

The procedure is performed by applying a medical teeth-whitening gel containing 32% hydrogen peroxide to the outer surfaces of the teeth. The gel is activated with a special lamp producing light of a certain wavelength. Active oxygen breaks away stain molecules in tooth enamel. Active oxygen has no harmful effect on dental enamel which therefore remains unchanged.

Only the best systems can activate the gel without drying out the enamel which contains 4% water. Due to the short half-life of hydrogen peroxide decomposition into active oxygen and water, the whitening gel should be reapplied to the teeth every 15 minutes.

The results of internal tooth bleaching are visible immediately after the gel is removed. You can expect to see the final tooth shade within two weeks after the whitening treatment.

At-home teeth whitening

At-home teeth whitening is performed by using a brush, with which bleaching gel is applied to the outer surfaces of teeth, and a tray. The bleaching gel is applied with the tray that is custom made to fit your teeth with precision.

Trays are inserted into the mouth for a few hours during the day or for several hours overnight. The bleaching gel for at-home whitening contains 16% or 10% carbamide peroxide which is safe to use without dentist supervision.

At-home whitening kits should be used for at least 14 days straight in order to obtain some visible results, and it is crucially important that all the instructions are strictly followed.

Which teeth whitening method is the best?

The best method of teeth whitening is a combination of both methods and daily use of whitening toothpastes.

The maximum effect is provided with in-office bleaching. To maintain the new tooth shade, at-home teeth whitening should be performed by using trays and a bleaching gel. For day-to-day maintenance of the tooth colour and for enamel restoration, it is recommended to use a dye-free whitening toothpaste containing 30% xylitol, 15% hydroxyapatite, and 1100ppm fluoride.

What is the best way to take care of your natural tooth colour?

Below are some useful tips that will help maintain your teeth’s natural colour:

  • avoid the consumption of food and beverages containing artificial colorants
  • stop smoking
  • maintain good personal oral hygiene
  • seek regular professional oral hygiene treatments
  • use safe at-home whitening gels
  • get professional (in-office) teeth whitening
  • check your teeth colour regularly by means of special dental-colour measuring instruments

Enamel demineralisation represents the first sign of dental caries. In order to ensure protection against caries, patients are advised to use liquids that:

  • contain active ingredients such as amine fluoride and sodium fluoride,
  • contain no colorants and
  • are alcohol free

Indications for use:

  • prevention and treatment of dental caries
  • tooth enamel protection
  • promoting tooth enamel remineralization
  • reducing enamel solubility
  • providing effective protection in hard-to-reach areas

Tooth enamel is the most mineralized tissue in the body and in the living world in general. It consists of millions of crystals (86%). The cells make enamel even before the tooth comes out. Its role is very important as it protects the dentin, allows chewing and biting, has an aesthetic significance and also affects our speech. It is the enamel that gives the tooth the right size, shape and colour.

There are several factors that promote the development of caries and lead to the damage of enamel and dentine. Enamel demineralisation is a precondition for the formation of dental caries. Early stages of tooth enamel demineralisation do not represent an irreversible injury and can be easily fixed. High-concentration fluorides are very effective in preventing and reducing the initial enamel demineralisation, which is particularly important in the areas that are more difficult to reach for cleaning, mainly in the pits on the biting surfaces of the posterior teeth and in the interdental spaces.

In the initial stage of demineralisation, a white spot forms on the enamel. When enamel becomes demineralised in its entire thickness, the “enamel gap” opens and changes occur in the underlying dentine. When bacteria enter or settle on the damaged area, we are talking about dental caries.

Carious damages are irreversible because the dentine does not regenerate and enamel restoration is no longer possible. Dental caries must be removed and the tooth damage replaced with a dental filling material.

In addition to a thorough maintenance of oral hygiene, it is also recommended to use an antiseptic liquid containing the active ingredient 0.2% chlorhexidine.

Indications for use:

  • periodic use to prevent gingivitis

  • use in the treatment of gum inflammation

  • use in a weakened immune system

  • providing effective protection in hard-to-reach areas

Chlorhexidine is the most effective antiseptic for reducing the amount of harmful bacteria in the oral cavity. It absorbs onto the tooth surface and oral mucosa, therefore it remains active for prolonged periods of time (prolonged effect). As a result, the plaque amount is reduced and gum inflammation (gingivitis) is prevented. By binding to bacterial cell walls, chlorhexidine causes bacteria to disintegrate and die. Chlorhexidine works only if it is bound to the tooth surface. Dental plaque, however, negatively affects and reduces its function.

Chlorhexidine may cause external discoloration (stains on the tooth surface). Pigments found in tobacco, food and beverages can become linked to chlorhexidine molecules which are already attached to teeth and other surfaces, therefore they become stained with these pigments. Sometimes, stains can only be removed through professional cleaning in a dental office. Chlorhexidine may cause certain side effects such as temporary taste loss or taste impairment, a mucosal erosion, and parotid gland swelling. These side effects are rare and transient, and they disappear after chlorhexidine is discontinued.

Chlorhexidine is prescribed for acute and chronic inflammation of the oral mucosa and after oral surgery procedures. Chlorhexidine is used, along with otherwise good oral hygiene, to prevent the inflammation of periodontal tissues. It should be taken periodically, every 3 months for 14 days. The preventive action of chlorhexidine is more effective than the curative one in cases where the inflammation of the periodontal tissues has already occurred.
